Hi everyone-

Does the ACA have a sample physical form available?

I am on a camp committee for my camp, and we are in charge of the physical and I would like to update it. The forms have been the same for at least the last 13 years.

Does your camp use the same form for staff/volunteers and campers?

Yes.

I am not sure where to find them though. I know that our Health Center gets them right from ACA.

The campers and staff have the same forms, though the campers have an extra one where their parents have to sign off.

My camp also uses a Campminder form that is online. We fill out all of the basics, and then there is just one form the doctor has to sign. That makes it easy on our end because if you don't have any major health issues, you don't have to jump through all of the hoops each summer.

My camps recently have used an online camp staff medical form. My most recent camp doesn't require much of a physical. It's basically a certification that you're in good enough health for camp activity, height, weight, and blood pressure. The new camp I'm going too does the health check by the health center.

The camp I am at, all the campers have major medical issues-- its a camp fro adults with physical disabilities ( it varies from people with severe arthritis to muscular dystrophy and almost everything in between. So an online thing properly wont work at least for the campers. We will check into the ACA forms for next years camp
